Diabetic Shoe Program 

Advanced Regional Center for Ankle and Foot Care participates in the Medicare Therapeutic Shoe Program that allows individuals who are diagnosed with diabetes (and meet certain criteria) to obtain one pair of diabetic shoes per year. Most Medicare insurance plans will cover 80% of the cost. Secondary insurance may cover the rest, but the individual is ultimately
responsible for the amount not covered by insurance. Three pairs of custom shoe inserts are also included and covered by
Medicare for individuals who qualify. 

Patients

To take part in our diabetic shoe program: 

To take part in our diabetic shoe program: 

  1. Schedule an appointment with our office for your Comprehensive Diabetic Foot Exam (CDFE) with one of our podiatrists. 
     

  2. After your appointment, you will be given a shoe folder containing paper work that must be completed by the medical physician that treats your diabetes.***
     

  3. After your appointment, browse our large selection of diabetic shoes and make your shoe selection. 
     

  4. We will take care of ordering your shoes and submitting all necessary billing information to Medicare or your private insurance for payment.

    Please note: Patients are responsible for any co-pays and deductibles not covered by insurance.  
     

  5. We will schedule an appointment for you to return to our office to pick up your shoes. 

    Please note: ALL required paperwork MUST be completed by your treating medical physician PRIOR to picking up your shoes. 

  1. Call or stop in any of our offices to receive a shoe folder.*** 
     

  2. Schedule an appointment with your physician of choice for your Comprehensive Diabetic Foot Exam (CDFE). 
     

  3. Have your medical physician complete all required paperwork pertaining to your CDFE. Make sure you also obtain a prescription for diabetic footwear. 
     

  4. Contact your medical physician that treats your diabetes and have them fill out all required paperwork pertaining to your diabetes. 
     

  5. Once ALL required paperwork is complete, contact our office at (814) 943-3668 to schedule a time to select your shoes. 
     

  6. We will take care of ordering your shoes and submitting all necessary billing information to Medicare or your private insurance for payment.

  7. We will schedule an appointment for you to return to our office to pick up your shoes. 

     

***Attention Patients and Non-Patients: 

Medicare and private health insurance REQUIRE ​that we receive the
following documentation prior to dispensing your shoes: 

  • A signed copy of your treating medical physician (MD, DO) most recent office note. ​
    This must have occurred within the past six months. 

     

  • A signed copy of your most recent diabetic foot exam note by your treating medical physician (MD, DO).
    Non-Patients: Advanced Regional Center for Ankle and Foot Care's diabetic foot exam note must be signed and approved by your treating medical physician (MD, DO).  

    This must have occurred with the past six months. 

     

  • A signed copy your the Statement of Certifying Physician from your treating medical physician (MD, DO).
    This must have occurred within the past three months 

Non-Diabetic Patients 

All are welcome to purchase a pair of diabetic shoes. 
For those that do not have diabetes, the full cost of the shoes
will be the
responsibility of the patient.
