When you schedule your appointment with us, we will ask for your insurance information. We will verify and check your insurance coverage prior to your appointment.
When you arrive for your appointment, please bring a valid insurance card with you.
Co-pays are due at time of service.
For patients with HMO plans, consult with your primary care physician and ask for a referral to Advanced Regional Center for Ankle and Foot Care for evaluation and management of your specific condition.
Because some insurances do require a physician referral, it is the patient's responsibility to obtain that referral prior to your appointment at our office.
Failure to have a valid referral may result in the rescheduling of your appointment.
